Lynx Appointments - Survey Intake
Swap the lightweight per-type intake questions for a full Survey when a booking needs real questionnaire power: pages, sections, scoring, conditional survey logic and the shared question bank.
Each appointment type chooses its intake mode. In "Survey" mode the native questions are hidden on the booking page; after the client picks a slot they are taken straight into the linked survey, and the survey response is attached back to the booking. The answers appear in the same place native answers do - the calendar meeting the booked resource sees, the confirmation email, and (with the CRM bridge) the opportunity - so the rest of the platform does not care which intake mode was used.

Per-type intake mode - each appointment type keeps native questions or switches to a Survey, side by side.
Linked response - a survey.user_input is created per booking and attached to it, so responses are auditable and reportable in Survey.
Answers everywhere - completed survey answers render into the meeting description, confirmation email and CRM opportunity through the shared renderer.
Post-booking hand-off - after choosing a slot the client is redirected into the survey; completing it refreshes the meeting the resource sees.
