The daily field-level risk assessment (FLRA) a worker fills before starting a shift — a carte de travail / analyse de risque sur le terrain — turned into one digital record per worker per shift, e-signed by the worker and the supervisor and printed as a card that matches the paper original.
Underground mining and other high-hazard operations run this card every shift: the worker lists the tasks (bons de travail) to do, answers a fixed set of risk questions with the supervisor, records the risk types and controls per task, verifies each workplace, and signs off. It is the primary proof that a hazard assessment happened before work started.
Key Features
Risk card per worker per shift — department, shift, tasks (linked to work orders), workplace description, tools, permits and equipment on one record, generated pre-filled for a whole crew in one action.
Field-level risk assessment — a configurable question catalogue (seeded with a 23-point set) answered by both the worker and the supervisor, with free-text answers where the question needs one.
Task risk register — risk types and control means per task, from configurable reference lists, plus the supervisor authorizations.
Workplace verification — the endroit / initiales / heure checks and the end-of-shift verification (cadenas removed, message to next shift, incident witnessed).
Worker and supervisor sign-off — the card moves worker-signed then supervisor-signed, each stamped with a signature and time.
Printable card — a report that reproduces the paper card, ready for a regulator, with the risk-type and control legends.
Integrates With
lynx_field_service — a risk card links to a shift, a crew and its work orders; a work order can require a signed card before work starts.
